Decorated veteran Jason Hill (Sean Patrick Flanery) is visiting his wife at the VA-33 hospital when a group of heavily armed terrorists, led by Adrian Rabikov (Weston Cage Coppola), storms the building. Their target is a top-ranking U.S. military general, but Jason's wife and daughter are also among the hostages.

Outnumbered and with his own psychological wounds from combat, Jason becomes the last line of defense inside the hospital. He must use his skills to take on the terrorists while the local police, led by Chief Malone (Michael Jai White), scramble to respond from the outside.
